If you are infected with COVID-19 or live with someone who tests positive, please follow the instructions below.

If you suspect that you might have been infected with COVID-19 or you are going to undergo PCR/antigen testing, please follow the instructions below. The latest form of Reporting of PCR/Antigen Test can be downloaded from the "Infectious diseases".
